The PSL is excited to be offering leagues again! In order to properly follow all CDC and PA guidelines, we have set up this form as a means of reporting all confirmed and probable cases of COVID-19.  In the event that participant in our leagues has a confirmed or probable test, we will ask you to complete the following:

Reporting a Confirmed or Probable Case

Any player who, determined through testing, has a confirmed or probable case of COVID-19 must immediately do two things:

• Notify the designated PSL COVID-19 Point of Contact: Greg Mitrik, PSL Director greg@pump.org 412-338-2133 x11
• Fill out the COVID-19 Confirmed or Probable Case Form below. This confidential form will be used for league notification purposes and none of the information given will be shared.
